Requesting Certificate of Insurance
It’s a new year & that means we’ve got a new COI Request Form link!
Please note that the link to the previous Certificate of Insurance Request Form is no longer valid. You will find the new address below and attached. The “BSA COI – Details” document is a handy guide you can share and keep on file.
Please share the following link with your team, staff &/or volunteers and keep on hand for reference:

If for an event/popcorn sale/camp
- Unit Type & Number
- District
- What is the event?
- When the event?
- Where is the event? (Business Name & Address)
- Certificate Holder: (if different from above)
- Any special requests? (Additional insured, etc.)
- Contact information/where to send COI
- Copy of contract, if applicable
If for regular/recurring meetings, etc.
- Unit Type & Number
- District
- That it is for Recurring Meetings/Activities.
- Where do you meet? (Name/Address of School, Church, etc.)
- School District/Parish (if applicable & if you know it)
- Any special requests? (Additional insured, etc.)
- Contact information/where to send COI
